Health Risk Assessments (HRAs)

A Health Risk Assessment is the first step in analyzing the health practices of your employees and is essential to any process of health improvement. Our comprehensive HRA is easy to complete, engaging, and gives participants tools they need for behavior changes. These results also allow our wellness professionals to assess an individual's readiness to change during their Face to Face health coaching sessions.

Benefits of our HRA

  • Identifies areas of greatest need for intervention
  • Acts as a catalyst for behavior change
  • Provides baseline data to track behaviors, risks, attitudes and change
  • Predicts future illness, early death and health care utilization
  • Enables the individual to evaluate change over time
  • Shows participants how their lifestyle affects their health

The HRA assesses the following self-reported information:

Health Designs

    Health

  • Current Health
  • Medical and Family Health History
  • Medical Self Care
  • Women's and Men's Health Questions

    Lifestyle

  • Tobacco usage
  • Nutrition
  • Physical Activity

  • Stress
  • Readiness to Change

    Biometric Measures

At the Health Screening event, participants receive a personalized report showing their health risks and overall wellness. Our HRA can be provided to your employees' onsite at your work location, online or via U.S. mail.

At the completion of the HRA, detailed group summary report is generated for the corporate client. This summarizes group health and safety practices, identifies costly risk areas, and highlights health promotion needs for planning purposes.
